Wrong color palette in Pinball Fantasies #30

This game changes the palette entries in mid-frame in order to select more colors than would be normally possible in the 640x480 16-color mode.

Add VGA per-scanline rendering
Closes #30. Many demos and intros now work as expected. This is an
addition to a per-frame rendering infrastructure inherited from Bochs,
so it's still inefficient and non optimized. More work necessary.
